Given a significant outbreak in your community [see details], develop a strategic Plan of Care that addresses the needs of families, groups, or communities. To promote the resolution of this community health crisis, follow the guidelines of the nursing process and address assessment, diagnosis, planning, implementation, and evaluation.

Step 1. Complete your Community Survey Tool
Follow Health Insurance Portability and Accountability Act (HIPAA) guidelines in your documentation, and record pertinent findings from the assessment.

Step 2. Analysis of the Community Survey (2-3 pages)
To analyze your assessment findings:

  • Prioritize the most pressing needs of your community. Provide a rationale for your prioritization.
  • Identify which needs respond best to nursing intervention and label these interventions as primary, secondary, or tertiary
  • Identify additional groups that will need to be involved in addressing the overall needs of your community

Step 3. Complete the Groups or Communities Care Plan Complete the Plan of Care chart below, using the data and pertinent findings from the community analysis. Remember, all care plan components must be traceable to the community analysis you completed in Step 2.

The Plan of Care should:

  • Analyze the community health crisis
  • Consider who should be involved in resolving the problem
  • List patient goals that are SMART (specific, measurable, attainable, realistic and time oriented)
    • two short-term
    • two intermediate term
    • two long-term care goals
  • Include two nursing interventions appropriate to the accomplishment of each of the identified patient goals*
  • Discuss the evaluative findings the nurse expects from successful interventions.
