Description
- Analyze the health care expenditures associated with the care of a defined population aggregate (such as diabetes, asthma, congestive heart failure, or other acute or chronic disease listed from Healthy People 2020) within your organization.
- Based on that analysis, review the literature to identify a best practice (a program or services) relevant to the care and disease management for the population aggregate.
- Following the business planning format provided in the texts (see below), develop a business plan (for a program or services) relevant to the care and disease management for the identified population aggregate.
Goals for and justification and rationale for program or services
- Market analysis and competitive analysis (what demand is there for the program or service, what are others in the community offering in this area and how does it compare to what your organization is offering)
- Include a marketing strategy for the program or service(s)
- Organizational Structure and Required management and staffing full time positions (include who reports to whom, even with administration)
- Financial analysis and budget (identify any assumptions; include operational budget and any capital budget needed; include expected revenue if applicable and a one year time frame in a narrative form; PLUS include a financial plan (Budget Matrix (Excel spreadsheet(s)
- Minimum of 10 references from a variety of sources including scholarly publications.
